Access Your Statement Online

What is the net amount of my residuals check? Did I get credited for each of the films I worked on? The answers to these questions and more are available to you online.

Sign up now for the Members "Private Area" of our website and you can access and review your annual statement online prior to receiving your check.

    1. Click the box SIGN UP FOR THE PRIVATE AREA from the Fund Home Page at: www.fmsmf.org.

    2. Click the box ON-LINE ACCESS AGREEMENT (you will need to print a copy, sign the agreement, and then mail the signed agreement to the Fund office).

    3. Scroll down to the bottom of the screen and complete the online application then click SUBMIT.
Upon receipt of BOTH the electronically submitted application and your signed Online Access Agreement, we will mail a PIN (Personal Identification Number) to you. Participants’ statements are posted on our web site about mid-June.
