f your address information has changed, it is very important that you inform the Fund of your current address. Simply do the following:

• Call and request a Musician’s Confirmation form be sent to you either via mail or fax.

• Visit our website at www.fmsmf.org and update your address online. (Note: This option is available only to those participants who have already signed up for the Private Area of our website).

• Send a written document containing your updated address information to the Fund office either via mail or fax (Important: This document is only valid with the participant’s signature and the date).

The Fund and its participating musicians are in a unique partnership. The more we communicate, the better the Fund can serve you. Please give us a call or email us with any questions.
